During your first consultation with Dr.Sam Kaldas you will be required to complete a new patient questionnaire. This form is available here. Please complete this form and email it to admin@baysideMSK.com.au or simply press the submit at the bottom of the form. This will save you time when you arrive.
You may also download the Patient Registration Form and bring it with you at your appointed time.
We also ask that you bring with you your Medicare and Private Health Insurance details, as well as copies of any medical imaging (X – rays, CT scans, MRI scans etc) that you have had done. We also kindly request that you have an up-to-date referral from your GP or healthcare provider which you may also email to the above address.
It is also advisable to wear appropriate and loose clothing to allow exposure of the body part in question. For example, if you have a knee problem, shorts or very loose pants is advisable which can be easily pulled up to expose the knee.
Please note that should you require assistance during your consultation, it is advised that you are accompanied by a family member or close friend who can remain with you at the clinic.
Once you have attended and undertaken your initial consult, Dr. Kaldas will advise you of when you can make a further booking to undertake the PRP process. This will involve three separate visits and will also involve you undertaking physiotherapy at the clinic to assist in your recovery process
